If you have not set up a Medical Practitioner online account, you must set one up before placing an order. If you have any questions, please call Medical Practitioner  support at 1-800-275-3633.

 Setting up a Medical Practitioner Online Account: 

1. Click the Medical Practitioner registration link and fill out the Medifast Provider Agreement form. (You may have done this in the past, but this must be done again to order online.)
2. After you have filled out the form and chosen your user id and password, a Medifast representative will approve your account within 24 hours.
3. Once your account has been approved, your user id and password will be activated.  You will receive an email confirming the activation of your account.
4. After the account has been activated, you will be able to log in to the Medical Practitioner store and order online.
